In preparation for your dental visits and to make the registration process efficient, we offer all the necessary forms online. Please print the necessary forms, fill them out, you can email them to cerritos@boghawaladds.com or Fax 562-860-6628 or just bring them with you at your appointment. At your first appointment please fill out New Patient Forms and Smile Analysis forms and bring it to us along with two Valid Identity Proof and your current dental insurance information. Providing us with accurate medical history information with a list of medications and dosage you are prescribed will helps us to better understand your health and its implications to oral health.
